To enroll, simply request an enrollment form and return with your payment.
Our CHOICE PLAN entiltles you to the following services at NO CHARGE:
- 2 comprehensive exams per year
- 2 cleanings per year
- 2 sets of bitewing (cavity detecting) x-rays per year
- 2 fluoride treatments per year (up to the age of 15)
You are also entitled to 20% off our regular fee schedule
CHOICE PLANS : enrollment period of one year (patient must be seen within the enrollment period or the plan is void)
$399 per year
Co-payment must be paid at the time of the service
All services must be paid in full or our regular full fee will be charged
